皮膚原位再生復(fù)原醫(yī)療技術(shù)在磷燒傷創(chuàng)面修復(fù)中的臨床應(yīng)用

摘要： 磷燒傷是熱力和化學(xué)物質(zhì)的復(fù)合燒傷,通常創(chuàng)面較深,嚴(yán)重者可達(dá)骨骼本院燒傷科2006年1月至2010年12月共收治磷燒傷住院患者248例,均采用皮膚原位再生復(fù)原醫(yī)療技術(shù)(MEBT/MEBO)治療,創(chuàng)面愈合好,所有患者均無功能障礙,臨床效果滿意治療要點如下:1.清除創(chuàng)面上的磷顆粒,避免與空氣接觸燃燒;2.在第1個24h內(nèi),每2h換藥1次,以盡快排除創(chuàng)面殘留的磷:3.次日,進(jìn)行藥刀結(jié)合,耕耘、搔刮創(chuàng)面,每6h清理創(chuàng)面更換MEBO藥膏1次,在去除殘留藥膏時再配合搔刮、耕耘創(chuàng)面,每日1次,連續(xù)3d;4.伴有吸入性損傷者,現(xiàn)呼吸困難或肺水腫時,盡早行氣管插管或切開;5.嚴(yán)格按照MEBT/MEBO規(guī)范進(jìn)行系統(tǒng)治療和局部換藥。
Phosphorous burns is a kiml of composite thermal and chemieal burns,the wound is generally much deep and may down to bone in severe case.A total of 248 cases of phosphorous burns were enrolled into our burns department between Jan.2006 and Dec.2010,and all were treated with MEBT/MEBO.The clinical therapeutic efficacy was good in that all the wounds were healed and no patient developed dysfumction.The key points of treatment are as follows:1.Remove the phosphnrus granules to avoid burning upon contact with air;2.During the first 24 hours,change dressing every two hours so as to remove the residual phosphorus as soon as possible;3.On the second day,use MEBO combined with surgical methods including incising and scraping the wot,nds,clean the wound and change MEBO every 6hrs.During the re-dressing,remove the remnant MEBO and serape and incise the wound once daily for three clays;4.For the cases of inhalation injury and complicated with dyspnea or puhnonary edema,perform trachea cannula or tracheotomy timely;.5.Strictly follow the standard procedt,res of MEBT/MEBO Lo perform systemic treatment and topical dressing change.
