MEBT/MEBO創(chuàng)面處理規(guī)范

摘要： 規(guī)范的MEBT/MEBO創(chuàng)面換藥技術(shù)可以減輕患者痛苦、縮短創(chuàng)面愈合時間、提高創(chuàng)面愈合質(zhì)量。創(chuàng)面處理原則為早期、全程、規(guī)范換藥,依創(chuàng)面深度不同而采用不同的處理方法。具體為:1.深Ⅱ度燒傷創(chuàng)面:創(chuàng)面愈合后使用疤痕軟膏聯(lián)合彈力繃帶加壓包扎1年左右預(yù)防瘢痕;2.Ⅲ度淺型燒傷創(chuàng)面:傷后即刻進行耕耘減張,大面積燒傷患者需在生命體征平穩(wěn)的狀態(tài)下才可實施耕耘減張?zhí)幚?但一般應(yīng)在48h內(nèi);創(chuàng)面愈合后使用疤痕軟膏聯(lián)合彈力繃帶加壓包扎1年半左右預(yù)防瘢痕;3.Ⅲ度深型燒傷創(chuàng)面:在傷后24h內(nèi)進行耕耘減張、薄化處理,耕耘減張后第2天對創(chuàng)面進行開窗,逐步清除壞死組織。創(chuàng)面愈合后使用疤痕軟膏和彈力繃帶加壓包扎2年左右預(yù)防瘢痕。在規(guī)范的MEBT/MEBO治療下,創(chuàng)面的愈合標(biāo)準(zhǔn)為深Ⅱ度深型以下創(chuàng)面無瘢痕愈合;淺Ⅲ度創(chuàng)面無瘢痕或表淺瘢痕愈合,無功能障礙。下圖是Ⅲ度深型創(chuàng)面規(guī)范換藥的典型圖例,詳細(xì)病例資料請觀看本期雜志所附光盤。
Standardized dressing change techniques of MEBT/MEBO can relieve patients'pain,shorten the healing lime and improve the wound healing quality.The principles of wound management are early application of MEBT/MEBO throt,gh the whole treatment course,following standard procedures to change dressing and adopting different management approaches based on wot,nd depths.And the details are as follows:1.DeepⅡdegree burns:after the wound healing,pressuring bandage the wound by using elastic bandage after applying MEBO Scar Ointment for about one year to preven! scar formation; 2.Ⅲdegree superficial burns:skin incision and tension relieving technique should be performed immediately after the injury,but for the extensive burns,it should be adopted under the conditions of stable vital signs,anti generally within 48 brs. After the wound healing,pressuring bandage the wound by using elastic bandage after applying MEBO Scar Ointment for about one anti a half years to prevent scar formation;3.m degree deep burns:perform the skin incision and tension relieving technique and the eschar thinning technique within 24 hrs after the injury..And at the second day after that,perform the windowing technique on the wound to remove the necrotic tissues gradually.After the wound healing,pressuring bandage the wound by using elastic bandage after applying MEBO Sear Ointment for about two years to prevent scar formation.Under the premise of standardizing application of MEBT/MEBO,the evaluation criteria of wound healing are that the wounds below deep 11 degree deep type heal without scar formation and the wounds of superficialⅢdegree heal with no scar or superficial scar but with no dysfunction.The pictures below are the typical cases of deepⅢdegree burns treatment following the standtard dressing change procedures.As for more case data,please refer to the attached DVD contained in the Journal.
