SLIPA喉罩在急診心肺復(fù)蘇困難插管中的應(yīng)用

摘要： 目的探討SLIPA喉罩在急診心肺復(fù)蘇困難插管中的臨床應(yīng)用價(jià)值。方法肺復(fù)蘇困難插管患者108例,按隨機(jī)數(shù)字表法分為L組、S組和Q組各36例。S組采用SLIPA喉罩置入,L組采用標(biāo)準(zhǔn)型喉罩(standardlaryngeal maskairway,LMA)置入,Q組采用氣管內(nèi)插管。比較3組一次置入成功率、置入時(shí)間、胃內(nèi)容物誤吸率和心肺復(fù)蘇成功率。結(jié)果①一次置入成功,S組、L組一次置入成功率均明顯高于Q組(P<0.01,P<0.05);S組和L組比較差異無統(tǒng)計(jì)學(xué)意義(P>0.05)。②置入時(shí)間,S組、L組置入時(shí)間均明顯短于Q組(P<0.01,P<0.05),S組明顯短于L組(P<0.05)。③心肺復(fù)蘇成功率,S組、L組心肺復(fù)蘇成功率均明顯高于Q組,S組和L組比較差異無統(tǒng)計(jì)學(xué)意義(P>0.05)。④胃內(nèi)容物誤吸率,S組胃內(nèi)容物誤吸率明顯少于L組和Q組(P<0.05),L組和Q組比較差異無統(tǒng)計(jì)學(xué)意義(P>0.05)。結(jié)論與氣管插管和LMA比較,SLIPA喉罩操作更加簡(jiǎn)便,能夠明顯降低胃內(nèi)容物反流誤吸的危險(xiǎn),且價(jià)格低廉,值得在臨床推廣應(yīng)用,
Objective To investigate the value of SLIPA laryngeal mask for difficult intubation in the emergency cardiopulmonary resuscitation in clinical applications.Methods A total of 108 cardiopulmonary resuscitation patients with difficult intubation were randomly divided into L group,S group and Q group with 36 cases in each group.SLIPA laryngeal mask airway insertion was used in S group,standard laryngeal mask airway (LMA) insertion in L group,and endotracheal intubation in Q group.The intubation success rate,insertion time,gastric content aspiration rate and success rate of cardiopulmonary resuscitation were compared among the 3 groups.Results ①The placement success rate of S group and L group were significantly higher than that of Q group (P<0.01,P<0.05); There was no significant difference between S group and L group.②The insertion time of S group and L group were significantly shorter than that of Q group (P<0.01,P<0.05),and S group was significantly shorter than L group (P<0.05).③the success rate of cardiopulmonary resuscitation; in S group and L group were significantly higher than that of Q group,S group and L group showed no significant difference (P > 0.05).④The rate of aspiration of gastric content in S group was significantly less than the L group and the Q group (P<0.05),L group and the Q group showed no significant difference (P > 0.05). Conclusion Compared to tracheal intubation and LMA,SLIPA laryngeal mask operation is more convenient,inexpensive,can significantly reduce the gastric contents aspiration risk,and it is worthy of clinical application.
